Concierge Geriatric Care Management
Weekly in-home wellness checks for seniors by a registered nurse, including health assessments, medication review, and coordination with your physicians.
Luminara Private Health helps older adults and their families manage ongoing health needs at home in Northern Miami-Dade and Broward County.
A nurse who knows your health and follows through.
Managing health at home involves more than attending appointments. Symptoms change, medication lists become complicated, and families need clear updates.
I visit weekly, follow changes over time, and help communicate concerns to your medical team. You work directly with me, so your care management stays consistent and personal.
What your care includes:
I visit you at home each week to assess your health, check vital signs, and review symptoms. Consistent visits help me understand your usual health patterns and recognize changes that need follow-up.
I review your medication list and available lab results, track concerns, and communicate significant health changes to your physicians. Your plan determines the level of additional support with appointments, prescriptions, and ongoing provider coordination.
With your permission, I keep family members informed about your health, changes I notice, and next steps. Whether they live nearby or out of state, they have a clearer picture of how you are doing. Update frequency depends on your plan.
Care Between Appointments Matters
I provide weekly RN wellness checks, symptom tracking, and care coordination to help recognize health changes and communicate concerns to your physicians. You and your family have a nurse who knows your health and follows through between appointments.
